naviHealth Inc (Brentwood, TN), a Cardinal Health (Dublin, OH) company, entered into an agreement to acquire Curaspan Health Group Inc (Newton, MA), a privately held provider of care-transition tools for hospitals and post-acute healthcare providers. Approximately 20 percent of all acute care hospital discharges in the U.S. rely on Curaspan’s platform to transition smoothly into post-acute care. Complementing EMRs and clinical workflows, Curaspan creates an integrated communication hub for otherwise siloed providers, enabling seamless patient referrals between sites of care. The transaction is conditional on regulatory approvals and other customary closing conditions and expected to close before the end of June 2016.
